For Parents / Guardians: I understand my minor child(ren) will serve in a volunteer capacity with The Bridge For Community Services. We have both read and understand The Bridge/FCS Volunteer Guidelines and Expectations provided for me and my child. I understand that due to the nature of the population served, many of whom are homeless individuals, The Bridge/FCS does not know the background or history of the individuals served or on our premises. I understand that I am responsible for my child’s well being and safety while serving with The Bridge/FCS and that I (or an authorized adult appointed) am required to keep my child within arm’s length at all times while volunteering and behind the designated assigned area. I have been informed of the liability and safety concerns and will not hold The Bridge/FCS responsible or liable for my child while he/she serves as a volunteer. I authorize my child to serve as a Minor Volunteer with The Bridge/FCS.

Photography Release

I grant The Bridge For Community Services (The Bridge/FCS), its representatives the right to take photographs, video or audio of me and my property while serving with them. I authorize The Bridge/FCS, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I understand and agree that any audio and/or visual material using my likeness will become the property of The Bridge/FCS and will not be returned. I acknowledge that since my participation with The Bridge/FCS is voluntary, I will receive no financial compensation I agree The Bridge/FCS may use such visual and/or audio material of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, fundraising, educational purposes and Web content.
